Every payment in American healthcare is the output of a rule. Almost nobody checks which one.Why does one blood test carry eight prices in the same building? Why was your claim denied - and is the denial even right? The Number Nobody Checks explains how American healthcare payment actually computes: fee schedules, DRGs, denial codes, appeal clocks and price-transparency files, with every worked example drawn from public federal sources the reader can verify. The author spent twenty years on the payer side - claims operations, core platform implementations, payment integrity - then rebuilt the federal payment systems from their published files and reconciled them against the government's own pricers until they agreed to the cent. The last disagreement left was thirty-eight cents. On the way down: a fourteen-hundred-dollar-a-claim error that nothing had ever flagged.Inside:• One hospital's own published prices for one tube of blood, eight rows apart• Two current federal files that disagree about the same vaccine, both correct• A parity ratio that reverses when you change the denominator• The five systems every claim passes through, and which one actually moves the money• A working desk reference: 57 denial codes with cause, fix, prevention and recovery outlook, appeal windows for ten payer types, and a one-page single-claim checkFor revenue cycle and denials teams, payer operations, provider contracting, benefits leaders, policy readers - and anyone who has ever stared at a remittance wondering where the number came from. The rules are public. The files are downloadable. The arithmetic is arithmetic. The number is checkable - this book shows you how.
